contribute to the research fund online. Just complete the information in this
form. Your credit and renewal information will be sent to Authorize.net a
secure server and will be processed there. Your credit card will be charged
with the applicable U.S. dollar fee listed in the dues statement you received
from the ASLMS office and any additional amount donated to the research fund.
